Every state has an Immunization Information System (IIS).
The IIS is a confidential, digital database. It keeps immunization records for children and sometimes adults. These records come from participating healthcare providers. To find your family members' records, ask your healthcare providers if they participate in your state’s IIS. If they do, they should be able to print out your official records.
What can you do if your provider does not have all of your family members' records? Use the IIS to see if they have your records. Start in the state(s) where you and/or your child received your last vaccines.

Information in an IIS is different in every state, but most contain at least the following information:
- Name (first, middle, and last)
- Birthdate
- Gender
- Birth state/country
- Parent's name
- Types and dates of vaccines given
