Up-to-date immunization records (also known as shot records) for your family, especially your children, are important. You will need your children’s official records to register them for:
- School
- Childcare/daycare
- Sports teams
- Summer camps
- International travel

If you move or change doctors, having an accurate record might prevent your child from needing to get vaccines they already had.

When:
An immunization record should be started when your child receives their first vaccine and updated with each vaccination visit.
How:
Ask your local health department or healthcare provider for your child's immunization record.
Make sure you keep up-to-date personal immunization records. Ask the doctor or nurse to record each vaccine given on your child’s record. Also, write down the name of the doctor’s office or clinic.
Keep your child’s record in a safe place where you can easily locate it.
Bring the record to each of your child’s doctor visits.
